magento upsell missing first item everytime - magento

Magento ver. 1.7.0.2, Up Sells show on the product page but never the first product I add as an up sell. For example if I ad a,b,c,d as up sell products, and yes I am reindexing, I go check the product page and only b,c,d are displaying. It's not a specific product issue as it happens the exact same when mixing and matching simple & configurable products and testing with different items.
It seems to be like somewhere an array is starting at one instead of zero, any ideas?

Magento configurable product not showing all posible combinations

I am using an attribute set for all the jeans products which have waist size, inseam and color. I managed to add the products to the configurable product but now all the options don't show in the front end. I already add all the options to the attributes and also linked the single products to the configurable product. The first product I created shows the correct attributes but the second one doesnt, instead it shows exactly the same options than the first product even when the extra options were already added as single products.

01) check the quantity in all simple products and make it as in stock
02) after changed go to configuration and re-index

Limit cart to 1 product per attribute/type - Magento

Our company sells pharmaceuticals online and by law we are limited to how much of a particular drug a customer can purchase.
For singular products with one brand this is easily controllable through default Magento functionality, my problem arises when we have different brands of a particular drug.
In a nutshell I can currently limit a customer to 1 pack of paracetamol from brand A but there is nothing to stop them getting another pack from brand B at the same time.
I would like to be able to check the cart for products with a particular attribute and limit them to 1 per cart. Ideally this would be when the customer clicks add to cart and the message would be displayed via the default Magento alerts that we currently have.
EDIT: I think the easiest way to solve this would be to check the SKU codes currently in the basket when adding a product to the cart. If there is a match, Throw up an error else add the item to the cart.

You can set maximum allowed quantity in cart from admin panel. System->configuration. From left tab see catalog->inventory. Set Maximum qty allowed in shopping cart to 1.

Price in Cart is not correct Magento 1.8

Today I've run into a new problem and can´t find a solution.
I've added some simple products and configurable products via MagMI. Everything is fine. Products are shown in the front-end as I want it. But following happens:
simple product 1 has a price of 49€, simple product 2 has a price of 90€.
Both are part of the same configurable product which has a price of 49€. Now, in cart, the single-price of both simples is 49€. (If I print out $_item->getSku() I get the right skus of the simple products, so I think, the products in cart are the correct ones)
How can I fix this?
It would be also helpful if someone could tell me where the subtotal is built exactly - I can't find this.
I would be thankful for all help or hints.
If the simple products are not visible individually then the price shown will be the price of the configurable product. The pricing variations on the associated single products are managed in the Associated Products tab of the configurable item.
Pricing is managed here as a plus/minus relative to the price of the main configurable.

Magento category page displays no products

When I view a category page (ie. url.com/category.html) the page shows the category title, description but no products. Doesn't seem to be loading them since I get this message "There are no products matching the selection."
The products are enabled, visible in catalog & search, have quantity and are in stock and product categories are listed under a root category called "Default Category". I have a featured products listed on the home page by category ID which display fine and individual product detail pages display as they should as well.
I have tried refreshing cache/indexes and searching Google all to no avail. I thought it may be an issue caused by a customization I made to the theme but switching to the default theme made no change so it would seem I must be missing something in the product or category configurations.
Possibly related? – searching always returns no results even if by product name
I am using magento ver 1.5.1
Please check following settings are done for your product
The products must be Visible in Catalog.
The products must be Enabled.
Product must have a stock Quantity.
The product must be set to In Stock.
If the product is set not to track stock, it still need to have a stock Quantity and be set to In Stock.
The product must be assigned to the target Category.
If using multi-website mode (or if you imported the products through Data Flow), the products must be assigned to the target Website.
You must refresh your “var/Cache” & rebuild all indexes from admin > system > index management

Next time if you have same problem you can try two things:
1 - Reindex catalog and products
2 - Check user permissions
If you move your database from one server to another or you restore the database you might want to check the customer_group table. More specifically you should check the "NOT LOGGED IN" records and its ID, it should be "0" if it is not the products won't be visible for that group of users which is pretty much all users on most Magento shops.
How many attributes do you have? Too many attributes marked as "Visible on product listing page" can break the indexes. Are there records in the catalog_product_flat_* table(s)?
OK after I looked up the records and replaced my theme's list.phtml file with the default one (for about the 100th time) products started displaying. Not sure what changed but as long as they continue to display I'm happy.

Magento Product Qty in stock but not showing in drop down

I have a configurable product which contains a number of simple products (each for a different size of product). Each simple product has an inventory value against it, but for some reason, one particular size, although it has an inventory value of 9, does not appear on the drop down on the front end.
All the other sizes do for the same product and I've compared them to ones appearing and can see no difference. I've cleared caches, updated indexes, etc. but nothing. Can anyone help?
Try opening the Configurable Product and checking that all of the simple products associated with it are added.

The simple product that's not showing in the front end is most probably missing from the group.
