In your IObjectWithSite::SetSite() implementation find and hook a sink object to the DWebBrowserEvents2 connection point of the browser object passed into the SetSite function. Now in your event sink's IDispatch::Invoke implementation check if the URL parameter is a string ends with PDF.
If you do not want to show the PDF at all, you probably want to consider handling the PDF MIME type by yourself.

It appears that, after opening an Outlook message using OpenSharedItem, you can successfully QueryInterface for IOleObject. But after that, I can't seem to successfully call any IOleObject methods.

MSG file format is well-described in MSDN. So, you can read the content without any third-party components or interfaces. To display the content (I guess the message body) you can use any web browser (for example, WebView2). The message body (HTMLBody) is represented by a regular web page. Most probably you will also need attached files that are used in the body.
Another possible alternative to use Extended MAPI for getting item properties. For example, you can use any third-party components for simplifying the work - Redemption.

In summary how can I programmatically determine if the sagepay 'card details' form which is injected into the sp-container (hosted in an iframe) is not valid for submission/tokenisation, when a call ‘tokenise’ link is used?
For example when the user has not completed all the required fields; Name, Card Number, Expiry or CVC.
I'm following the flow calling sagepayCheckout.tokenise() when the user clicks our 'Complete Payment' button (referred to as the tokenise link in the custom flow example).
NB: Implementation is part of a SPA app in my particular case I'm not submitting the (outer) form rather handling things using javascipt and calling the tokenise method.
Normally the onTokenise call back method is called on success or when wrong card details have been entered or merchantKey has expired. But when the injected sagepay form fails its own validity the onTokenise call back is never called.
NB: Due to browser security I can't check the state of the fields inside the iframe.
Ok some good points to note:
The sagepay form does feedback to the user about errors that required fields need to be completed.
And sagepayCheckout.tokenise() doesn't try to submit my (outer) form when these errors exist.
But there doesn't seem to be a programmatic way to know if the required fields need to be complete (or the onTokenise call back wont be called).
The main reason I need to know when the form is incompleted or tokenise has failed would be to re-enable our 'Complete Payment' button. Currently our button is disabled as soon as the user clicks it to prevent multiple clicks.
I am having this exact same problem. The only way I have been able to think of to get around this is to use the message event listener on the window. The sagepay.js library registers a 'message' event on the window object probably when the sagepayCheckout method is called.
Side note: this method is also poorly implemented as anything else that uses the window message event will cause the sagpay.js library to throw errors.
I think the only purpose of sagepay.js' message event hander is to resize the iframe window when its content changes. If you were to assume the content of the iframe will only change when validation fails you might be able use that to reset the state of your 'Complete Payment' button.
I would also suggest making a complaint to Opayo. This library is clunky and in desperate need of improvement.

You can install a PassThru App and read the HTTP header. Then you can parse the header text to get the MIME type, and good luck with that.

I need to save an image on a client sent by the server, but I do not know how.
This activity must be done in a transparent manner without affecting the existing operation of the application.
From the tests done I get this error:

the image is generated on the server, but not for direct client request (there is no link to click on web page), the picture is composed using other services on the Internet.
reconstruct the image on the server.
This image must be sent to the client to be saved locally.
so I'd like it to appear a window where you assign the destination image
plus I'd like the rest of the application were not affected by this activity.
The application is yet on production.

In other words, you were trying to mix the binary data of the image with the character data of the HTML output, or you were trying to do this in a JSP instead of a Servlet. This is indeed not going to work. You need to send either the image or the HTML page exclusively in response to fully separate requests.
In your JSP/HTML page just have a link to the image, like so:
click to download image
Then, in a servlet listening on an url-pattern of /imageservlet/*, you just get the image as InputStream from some datasource (e.g. from local disk file system as FileInputStream) and then write it to the OutputStream of the response the usual Java IO way.
You only need to set at least the Content-Disposition response header to attachment to make sure that the client get a Save As popup dialogue, else it will be displayed straight in the browser. Setting the Content-Type and Content-Length are also important so that the browser knows what the server is sending and can predict how long the download may take.

Note: you cannot control where the client would save the image, this would be a security hole. This way websites would be able to write malicious files on client's disk unaskingly.
Update: as per your update, there are two options:
You need to let the client itself fire two HTTP requests (I've answered this in your subsequent question)
Create a client side application which does all the task directly at the client side and then embed this in your webpage, for example a Java Applet. With an applet you have full control over the client environment. You can execute almost all Java code you'd like to execute and you can write files to disk directly without asking client for the location to save. You only need to sign the applet by a 3rd party company or the client needs to confirm a security warning before running.
Its up to the browser how all types of output are handled. Web pages are given a content type of html which the browser understands and ends up rendering ass a page that we can see. Images are given content type of image/jpeg etc which are rendered as images when in a page etc. To force a download prompt one needs to use a content type of a binary file rather than that of an image so the browser forces the download rather than shows the image. To ensure this use something like "application/octetstream"... i cant recall exactly but its easy to google for.
How would you suggest working with files that is stored on the note of a entity in Crm. Could you write a generic method that will enable you to access any type of file? Or would it be better to have a method for dealing with each type of file?
For example, we are going to be saving a mix of swf files and xml files on the entity, so would it make sense to have a method each for example:
GetXmlFilesOnAccount(accountid)
GetSwfFilesOnAccount(accountid)
When you upload an attachment to CRM the mimetype is also saved as part of the record information.

The post is missing the actual query needed to retrieve the annotations but you can tell what columns are required from the method signature.
My suggestion using your methods:
* GetXmlFilesOnAccount(accountid)
* GetSwfFilesOnAccount(accountid)
Retrieve account activitypointers by regardingobjectid(in your case accountid guid)
Loop through returned activitypointers
Get attachments for each activitypointer (activitypointer.activityid = activitymimeattachment.activityid)
Store attachments (disk, etc)
You don't even need two methods. You can retrieve all attachment file types for a given note (annotation) with a single method.
