I have imported the rewrite rules from .htaccess file:

However when I request http://appname/page/param it is rewritten to http://appname/appname/page/param.
At IIS Manager Rewrite Rules it says "Input URL path after /appname". In my opinion it should be "Input URL path after /", but I do not know how to change. May anyone help, please?
Did you try your hunch? You're right.
The second parameter of a RewriteRule is the substitution, and should be a real path for an internal rewrite. If your index.php is in the web root, and there's no appname directory there, You can remove the appname/ from your substitution.

In this case I need that whenever the URL is "/box/camp" there is access to "/_box/access_box_files/cam" and that all the information that comes in front of "(.+)?" "$1" is included. Without changing the URL in the browser, or forwarding the user.

online i have a problem:
i put this file on subdomain jcore.miodomio.com and it rewrite URL but not work
example:
without htaccess it's work:
jcore.miodominio.com/index.php/en/home
with htaccess
jcore.miodominio.com/en/home
Not Found
The requested URL /en/home was not found on this server.
can anyone help me? suggestion?

Try change your htaccess to:

Somethings to consider.
Did you enable Apache rewrite module?
If enabled check whether your virtual host file is configured properly So to make sure its hitting .htaccess or try putting some dump on .htacess , So if its hitting .htaccess than you should get an 500 server error.
If you are not getting error than its mostly the problem in your host settings.
I have a website design prototype coded in Code Igniter 2.0 and I can't access the internal pages.

Problem is due to absence of .htaccess file or incomplete .htaccess file.

When I put an URL as below it's not working and gives page not found error

But it works with

I don't want to show index.php in my URL, it should be http://www.reviewongadgets.com/latest-mobile

The most common solution to this problem is usually the missing question mark ? after index.php in .htaccess, so
RewriteRule ^(.*)$ index.php/$1 [L]
should be
RewriteRule ^(.*)$ index.php?/$1 [L]
E.G. on my Windows XAMPP, I do not need the "?", but in a Linux hosting environment it is usually required.

You might not have rewrite_module module loaded.

I have a drupal installation set up in a subdirectory of a domain (www.mydomain.com/cms), but I'm having trouble getting the queries to format correctly using mod_rewrite. I need url requests coming in as:

Where these rules you posted are located? If you create a .htaccess inside the "cms" directory with the following content, I believe it should work fine:
