How can I add a UML Activity diagram to a project in visual studio 2017?
You can't. Visual Studio is a code editor, not a UML modeling tool.
There has been some support for UML like diagrams in VS in the past, but this has been discontinued in the latest versions.

The uml sequence diagram was drawed in visual studio 2015, and it can still be opened in 2015. But double click the diagram file in 2017, the visual studio shows me xml instead of draw the diagram.

VSS is not included since VS 2010 due to performance issues. According to

you can choose "Microsoft Visual SourceSafe" plugin from Tools -> Options -> Source Control.
I suggest you to use TFS/CVS/Git/SVN rather than VSS for better version control of your application.
We just switched over to VS 2013 and I heard that you're supposed to be able to
generate code maps for your entire application. Awesome feature indeed, that could
get new developers on our project up to speed.
Watched a couple of tutorials, but when I tried to just right click on a method
in the application, the 'Show on Code Map' context menu is missing. In fact, I
can't find anything in VS that has anything to do with Code Maps.

I tried installing Modeling SDK for Microsoft Visual Studio 2013, but that didn't do anything.
Anyone got any ideas?
You need Visual Studio ULTIMATE to create Code Maps.

This has changed for Visual Studio 2015 and Visual Studio 2017:
The Enterprise Edition allows creating code maps.
The Community and Professional Editions can open diagrams generated in other Visual Studio editions in read-only mode.
I don't know whether it is missing in Visual Studio 2012 but I wonder why I cannot see the advanced tooltip feature in 2012.
In VS2010, once you hover your mouse over a code, it displays the following:
But it is how it looks right now in Visual Studio 2012:
And it looks rather primitive compared to Visual Studio 2010.
That's from a Visual Studio Plug-in. I think it's either PowerTools or VSCommands. You should install the same plug-in in VS2012, and you should get that behavior back.
(Just confirmed that it's Productivity Power Tools that provides that feature.)
